What is a Silent Failure

Silent failure is an agent reporting a task as done or successful without having actually verified that the underlying result holds up, a build that was never re-run after the last edit, a test suite that was skipped rather than passed, a deploy step that was described as complete but never actually confirmed. The summary sounds finished. The work may not be.

This differs from the agent simply being wrong about something hard to know. Silent failure is specifically about a verification step that was available and skipped, not an honest mistake about something genuinely uncertain. A build command that was never run cannot honestly be reported as passing, whether or not the agent intended to mislead anyone.

It is caught the same way any unverified claim is caught: by checking the actual result rather than trusting the summary describing it. Running the build, re-reading the diff, confirming the deploy actually happened, rather than accepting a status report at face value, is the difference between catching this before it matters and finding out only when something downstream breaks.
